The Mental Health Commission has published four reports into in-patient health centres, which detail non-compliance with areas such as privacy, the condition of premises and risk management.
Selskar House at Farnogue Residential Healthcare Unit outside Wexford town was found to be 81% overall compliant with regulations.
But there were issues over the ordering, prescribing and storing of medicines.
Hygiene was not maintained to support food safety regulations.
Where a resident required a therapeutic service that was not provided internally, the centre did not arrange for it elsewhere.
The Department of Psychiatry at Roscommon University Hospital was 75% compliant with regulations.

A lack of individual care plans were found in relation to several residents and not all administration of medications were documented for two residents.
The Eist Linn Child & Adolescent inpatient unit at Blackrock, Cork was found to be 78% compliant.
